Wellness Exam

A wellness examination is appropriate once the chronic illness has resolved for maintenance of condition, prevention of disease, and for those seeking optimum wellness. A thorough history, physical examination, and blood panel will be performed. If warranted, advanced lab panels may be ordered.

Financial Policy

Payment is due at time of service. Upon request, a superbill can be given to self-submit to insurance. Certain labs maybe billed directly to insurance. Credit card, cash, check, and health savings account are accepted. Please contact the office for further information.
